2. What should I wear to my appointment?
Please wear something comfortable that you can move freely in. Long shorts or sweat pants are best. Please no short shorts.
3. When should I arrive at my appointment?
Please come at least 15-20 minutes early.
4. What information is important for me to tell the physical therapist?
Important information is anything concerning your health; cancer, pregnancy, osteoarthristis.
5. What is your cancellation policy?
We realize that circumstances may cause you to arrive late or miss an appointment.
Late for an Appointment:
Please call if you know you are going to be late, so that we may try to rearrange our schedule to accommodate you or if necessary reschedule your appointment.
Missed or Cancelled Appointment:
Should you need to cancel, please contact us within 24 hours of your scheduled appointment. By doing so, you make it possible for us to see another patient whom might otherwise have had to wait to be seen.
ALL CANCELLATIONS WITH LESS THAN 24 HOURS NOTICE WILL RESULT IN A $65.00 FEE.
ALL MISSED/NO SHOW APPOINTMENTS WILL RESULT IN A $65.00 FEE.
6. Is my privacy protected?
The HIPAA Privacy Rule establishes national standards to protect individuals' medical records and other personal health information and applies to health plans, health care clearinghouses, and those health care providers that conduct certain health care transactions electronically. This requires appropriate safeguards to protect the privacy of personal health information, and sets limits and conditions on the uses and disclosures that may be made of such information without patient authorization. It also gives patients rights over their health information, including rights to examine and obtain a copy of their health records, and to request corrections.
